For self-evaluation only. This worksheet mirrors the point system, weights, and documentary-evidence requirements described in JC3 (2022) and its Enhanced Implementing Guidelines (2026). It is meant to help you gauge your standing before your Institutional Evaluation Committee (IEC) review — it does not replace the official Individual Summary Sheet (ISS), does not collect or upload any files, and final points always rest with your IEC/REC/EAC/CC and Governing Board.

Original instructional materials (peer-reviewed, IMDC-approved, in library holdings & listed in the syllabus) and academic programs developed/revised.

Evidence allowed only: Cover/title/copyright/TOC pages of the IM; IMDC (or equivalent) evaluation & approval-for-use record; syllabus listing the IM as reference, duly approved; librarian's certification that the IM is in library holdings; and, for co-authored IMs, the signed Certificate of Contribution Form_IM (Annex C) with dean's certification. For academic programs: certification signed by the Dean on role (lead/contributor), Academic Council resolution, Governing Board approval, and valid RRPA/COPC.

Adviser/panel service in special/capstone projects, undergrad/master's theses or dissertations (program must have valid RRPA/COPC), plus student mentorship/coaching.

Journal articles must be in a journal actively indexed in Scopus, Web of Science, or the ASEAN Citation Index at time of publication (or peer-reviewed, for qualifying Filipino-language journals).

For attendance/presentations: program, certificate of participation/presentation, and SUC President or Governing Board approval to attend/present (as applicable to local/international).
